Activity Summary
This session provides an overview of the various intravenous infusions commonly used to support and enhance perfusion in the pediatric population. Physiology of adrenergic receptor function and core concepts of inotropy and lusitropy are reviewed. Medications covered in this discussion include the alpha and beta-adrenergic agonists, milrinone, vasopressin and several vasodilators (nitroprusside, nicardipine, fenoldopam). Special emphasis is placed on the expanded spectrum of effects elicited by vasoactive drugs when titrated to different rates or levels. The dilemma of how to thoughtfully choose the best combination of vasoactive agents for any given patient based on hemodynamic status and presentation is also explored. Case studies conclude this session to highlight the main points of proper medication selection and dosing.
Objectives
- Define the concepts of chronotropy, inotropy and lusitropy as they relate to vasoactive medications.
- Describe beta-1, beta-2 and alpha-1 adrenergic receptor effects and identify specific vasoactive medications exhibiting these effects.
- Explain dose-related and agent-specific adverse effects of common vasopressor and inotrope infusions.
