Pain management in burn patients presents a major clinical challenge because of pharmacokinetic and pharmacodynamic changes, considerations for medication selection, and lack of a standardized approach. Therefore, a tailored approach to analgesia and sedation is an integral component of the intensive care unit (ICU) stay in this patient population. Background, break-through, and procedural pain should be addressed when caring for these patients.

Objectives

  • Discuss the pathophysiology of burn pain and deep tissue injury
  • Describe the changes in pharmacokinetics and pharmacodynamics in critically ill burn patients that alter absorption, distribution, metabolic
  • Compare the mechanism of action, dosing, and side effects for medications used for analgesia and sedation in critically ill burn patients
